Changing Lives has longstanding experience of supporting people who are victimised for exploitation, which began 15 years ago with our dedicated services for women with existing vulnerabilities
Key Activities
Provide one-to-one support for clients using an assertive outreach style of working, promoting choice, well-being, and the protection of all individuals. Lead on providing outreach support, having an awareness of personal and professional self-care & boundaries.
Identifying safeguarding concerns and escalating them as appropriate internally and externally.
Working with service users to develop and maintain support plans and records, including risk assessments and safety plans.
Recognise indications of risk such as substance misuse, domestic violence and sexual exploitation, and work in partnership with specialist external providers to reduce the risk of harm and abuse.
Support individuals to deal with complex relationships and provide appropriate advice and signpost where exploitation and substance misuse is an identified concern.
Working with individuals and relevant agencies to identify triggers that cause crisis whilst supporting management of these triggers to minimise further crisis.
Engage with specialist practitioners internally and externally to identify those at risk of custody and involvement with criminal justice system.
Support people to access a range of services to meet their individual needs, such as Probation, CRS, Substance use support, Housing, Domestic abuse services, Mental health, Sex worker support, and sexual health.
This may include attending appointments or drop-ins, making warm referrals, and collaborating with multi agencies to reduce disengagement.
To be aware of external service processes to safeguard individuals who are exposed to sexual exploitation and other risks. Support people to report sexual assault /rape and share intelligence appropriately.
Develop and maintain good working relationships with key organisations and their staff to support our clients access to the services they require. Support people to engage in community groups/activities to improve social, emotional, and financial wellbeing.
Be familiar with safeguarding legislation (Including Domestic abuse legislation) and processes, ensuring that any issues in relation to safeguarding are brought to the attention of the Project manager and appropriate action taken.

QUALIFICATIONS
NVQ Level 3 in Health & Social Care or equivalent appropriate qualification.
· Experience of working with people involved in the criminal justice system. Experience of working with individuals who are socially excluded. Experience of working with people who have experienced multiple compound traumas. · Experience of working with people who face challenges engaging with services.
· Experience of working with people where substance misuse has been/is a contributing factor.
· Experience of working with people involved in survival sex work or those vulnerable to sexual exploitation.
· Experience of assertive outreach or community-based working.
· Experience of managing risk, working within safeguarding procedures and working in a confidential environment
Experience of working in an environment that is health and safety aware Experience of working to set administrative guidelines.
